Event Registration Cancellation Form

Sometimes our schedule shifts and we need to cancel an event we have on our calendar. Should you need to cancel your reservation, please do so by the FINAL registration date of the specific event you are unable to attend. 

Cancellations made after the registration end date may still incur a fee to cover the cost of charges the chapter may still be held accountable.
